  • Mg per unit = concentration divided by 100

Reported examples included mistaking 5 units for 50 units, prescribing 25 units instead of an intended 5 units, and prescribing 20 units instead of 2 units.   • Whether the reaction follows one site, one preparation, or one lot

A clear vial is not proof of sterility or low endotoxin. Conversely, an unusual-looking vial raises a reason to pause, but appearance alone still cannot identify the contaminant or cause.
