The 2026 LEAN-PREP paper is a protocol, not an outcome study. It plans to randomize 232 adults for six months while they begin semaglutide or tirzepatide. Retatrutide is not included.
The planned resistance program uses three home-based sessions per week, progressing from one to three sets for major muscle groups. Protein supplementation targets 1.6 g/kg/day. Those numbers describe the study design. They are not a universal prescription and have not produced results yet.
The primary outcome is MRI-measured quadriceps cross-sectional area. Secondary measures include DXA body composition, strength, physical function, activity, sleep, and metabolic markers.
